Facility and Van Usage Request (Mackey or Owensville Campus)
If you are interested in using the facilities or vans of the Mackey Church of the Nazarene (at either the Mackey or Owensville campus), complete and submit the following form. The church reserves the right to pre-empt any reservation for the use of its facilities and property by its ministries, as deemed appropriate by the church board or its designees.
At the bottom of the reservation request form, you will be required to acknowledge that you have read and agree to follow the Christian Life Center and Banquet Room Policy and our Third-Party Vendor Policy before you will be able to submit this form. In addition, you will be required to complete and return to the church office the Mackey Church of the Nazarene Facility Usage and Hold Harmless Agreement before your reservation will be approved.
